What is a fractional CTO, and who is this for?
A fractional CTO is a senior technical leader who works with your company part-time and owns the decisions a full-time CTO would, at a fraction of the cost. In digital health the role differs from a generic startup CTO: the software has to hold up with clinicians, regulators, and patients, and the wrong early architecture is expensive to unwind. (New to the model? Read what a fractional CTO does.)
This engagement is for funded digital health companies that need senior technical leadership before a full-time CTO hire makes sense, typically when the product has to clear a clinical, regulatory, or enterprise bar and early architecture decisions will be hard to reverse.
When should you bring one in?
- Raising a round. Investors want credible technical diligence and a defensible architecture story.
- Pre-build architecture. You are about to make data-model, platform, and vendor decisions that are costly to reverse.
- First engineering hires. You need someone senior to define roles, set the bar, and interview for a regulated context.
- A regulatory or clinical requirement (SaMD, HIPAA, an eCOA or digital endpoint) that your current team has not navigated.
What do you get?
- Architecture ownership: platform and data architecture built for longitudinal, regulated clinical data, with audit trail, interoperability, and security in view from day one
- Engineering hiring: role definitions, leveling, interviews, and the first senior hires
- Vendor and build-vs-buy decisions with total cost and regulatory burden in view
- Regulatory posture: HIPAA and SaMD requirements, and the documentation trail reviewers expect
- External technical representation to investors, partners, clinical collaborators, and enterprise or pharma diligence
- A path to first production release, and a clean handoff to a permanent technical lead when you are ready
How does it run?
- AssessReview the product, codebase, team, and regulatory intent; agree on priorities for the engagement.
- Architect & hireSet the architecture and roadmap; define roles and build the engineering team.
- Ship & hand offLead through first production release, then transition to your permanent technical leadership.
We propose the smallest engagement that produces real value. Fractional CTO is one of our engagement models; many teams start with the fixed-scope Initial Audit.
Why a scientific counterpart, not just an engineer?
Digital health work falls into a gap: engineers ship software that does not clear the clinical bar, and clinicians design studies that cannot be implemented. The practice is led by Mike Hittle, a senior software architect (PhD, Epidemiology & Clinical Research, Stanford (2026)): lead architect of Stanford Medicine's mHealth Platform 2.0, founder of the open-source CardinalKit, and architect of the PHENOMS eCOA platform. You get one counterpart who holds the architecture, the science, and the regulatory posture.
Frequently asked questions
How much does a fractional CTO cost?
Engagements are priced as a monthly retainer or a set number of days per week, well below the fully loaded cost of a full-time executive. We scope the smallest engagement that produces real value on the first call and grow from there.
When should a digital health startup hire a fractional CTO?
When you are raising and need credible technical diligence, making architecture decisions that are hard to reverse, hiring your first engineers, or facing a regulatory or clinical requirement your team has not navigated.
What makes a healthtech fractional CTO different?
Digital health adds regulatory, clinical, and data-integrity constraints most engineering leaders have not worked under. We pair senior software architecture with doctoral training in epidemiology and clinical research, so one person holds the whole picture.
