The main public benchmarks
| Benchmark | Paper | Task | Size (per paper) | Metric | Limitations |
|---|---|---|---|---|---|
| MedQA | Jin D et al., 2020 | Multiple-choice board exam questions (US, Mainland China, Taiwan) | 12,723 English; 34,251 simplified Chinese; 14,123 traditional Chinese | Accuracy | Exam-style; public, static test set |
| MedMCQA | Pal A et al., 2022 | Multiple-choice entrance exams (AIIMS, NEET PG, India) | >194k questions, 2.4k topics, 21 subjects | Accuracy | Exam-style; India-specific; static and public |
| PubMedQA | Jin Q et al., 2019 | Yes/no/maybe questions answered from PubMed abstracts | 1k expert-labeled; 61.2k unlabeled; 211.3k generated | Accuracy (1k labeled set) | Small labeled set; research reading, not patient care |
| MultiMedQA | Singhal K et al., Nature 2023 | Six existing QA datasets (incl. MedQA-USMLE) plus HealthSearchQA, with a human evaluation framework | Aggregate of components | Accuracy plus human ratings | Mostly reuses exam/QA sets; human evaluation is costly to reproduce |
| HealthBench | Arora RK et al. (OpenAI), 2025 | Multi-turn health conversations graded against physician rubrics | 5,000 conversations; 48,562 rubric criteria; 262 physicians | Rubric score (model-graded) | Built by a model developer; model-based grading |
| MedHELM | Bedi S et al., 2025 | Clinician-validated taxonomy of real clinical tasks | 121 tasks; 35 benchmarks (17 existing, 18 new) | Task-specific; LLM-jury | Some tasks use restricted data; LLM-jury scoring |
Sizes are as reported at release; later versions and leaderboards may differ. HealthBench and MedHELM scores depend on model graders or juries and are not directly comparable to accuracy.
Contamination: why static public test sets age badly
Preprint evidence (not yet peer-reviewed): a 2025 preprint argues that benchmarks derived from public exams raise “contamination bias that can inflate performance” (arXiv:2510.09275 (2025 preprint)), and a 2026 preprint describes test sets that “inadvertently leak into training corpora, leading to inflated performance estimates” (LiveMedBench, arXiv:2602.10367 (2026 preprint)). Peer-reviewed work on leakage in machine learning generally found it in 17 fields and 294 papers (Kapoor S, Narayanan A, Patterns 2023); that study is not specific to medical LLM benchmarks.
Why custom evals matter
A systematic review of 519 studies of LLMs in health care found that only 5% used real patient care data, 95.4% used accuracy as the primary dimension, and calibration and deployment considerations were rarely measured (Bedi S et al., JAMA 2025). MedHELM’s authors note that near-perfect exam scores “inadequately reflect” real clinical practice. And locally, a widely deployed sepsis model scored an AUC of 0.63 on one health system’s data (Wong A et al., JAMA Intern Med 2021): performance has to be checked on your own population.
What it means for startups
- Use public benchmark scores as a first filter, not evidence of fit for your use case.
- Build a held-out eval from your own task, users and de-identified data, with clinician-defined rubrics; measure safety and calibration, not only accuracy.
- Keep the custom test set private and versioned, and re-run it when the underlying model changes.

Frequently asked questions
What is the difference between MedQA and HealthBench?
MedQA is multiple-choice board exam questions scored by accuracy. HealthBench is multi-turn health conversations graded against physician-written rubrics by a model grader.
Are medical AI benchmarks contaminated?
Preprint evidence suggests static public benchmarks can leak into training data and inflate scores; peer-reviewed medical-specific measurements are still limited.
Why build a custom eval?
Public benchmarks rarely use real patient care data or measure calibration and safety. A private eval on your own task and population tests what buyers actually care about.
